Helping Children With Intellectual Disabilities to Thrive
St. Luke’s Penn Foundation’s Supports Coordination program helps children and adolescents with intellectual disabilities access the services and support they need to grow, learn, and thrive. We partner with families to provide personalized, developmentally appropriate care that promotes safety, well-being, and long-term success.

Supporting Your Child’s Goals and Their Journey
Guided by the principles of Everyday Lives and Positive Approaches, we believe every child deserves the opportunity to make choices, be included in their community, and build a future that reflects their strengths.
Take the First Step
To begin services, contact your local County Administrative Entity (AE) to determine eligibility. Once eligible, families can choose a Supports Coordination Organization (SCO). St. Luke’s Penn Foundation is an SCO serving Upper Bucks County and Northeastern Montgomery County.
