St. Luke’s Penn Foundation’s Supports Coordination program helps children and adolescents with intellectual disabilities access the services and support they need to grow, learn, and thrive. Take the First Step
To begin services, contact your local County Administrative Entity (AE) to determine eligibility. Once eligible, families can choose a Supports Coordination Organization (SCO). St. Luke’s Penn Foundation is an SCO serving Upper Bucks County and Northeastern Montgomery County.
