St. Luke’s Penn Foundation’s Supports Coordination program helps adults with intellectual disabilities access the services, resources, and ongoing support they need to live safely, confidently, and as independently as possible.

Our team coordinates and monitors services while supporting individuals and families with education and guidance, always aiming to provide care in the least restrictive way based on each person’s needs.

Take the First Step

To begin services, contact your local County Administrative Entity (AE) to determine eligibility. Once eligible, individuals and families can choose a Supports Coordination Organization (SCO).
