REACH (Intensive Psychiatric Rehabilitation)

REACH (Intensive Psychiatric Rehabilitation) aims to assist individuals with mental illness and co-occurring substance use disorders in setting and achieving personal goals in their living, learning, working, and social environments. Services are provided on an intensive, time-limited basis through group work and individual meetings.

Five Phases

Readiness Assessment
Gauging a person's need/desire to set a goal for change and evaluating person's willingness to give up old ways of doing things & to take a leadership role in one's recovery journey

Readiness Development
Increasing self-understanding, learning recovery principles and community-based opportunities, and experiencing accomplishments

Goal Choosing
Identifying the dream job, home, or place to go to school and gathering information about identified goal options

Goal Achievement
Identifying critical skills and needed supported to successfully achieve the chosen goal

Goal Keeping
Evaluating and supporting the person in his/her chosen goal
