For Adults

Eligibility

  • A mental retardation diagnosis must be present prior to 22 years of age
  • Individuals must have a full-scale IQ of 70 or below
  • A psychological evaluation must utilize a standardized test in determining that the individual has been diagnosed with mental retardation

Process

  • Initial contact and determination of eligibility
  • Intake and assessment of needs
  • Development of annual services plan detailing services and supports needed
  • Locating and coordinating of supports and funding
  • Monitoring of supports
