Mental Health Case Management

Mental Health Case Management provides individualized supportive services to children and adolescents, ages birth through 17 years, using a psychosocial rehabilitation philosophy. The focus of services is to promote recovery from mental illness and/or co-occurring substance abuse disorders through the development of working partnerships with children/adolescents and their families that facilitate the achievement of desired life goals and roles.

Through linkage to resources, skills training, and advocacy, meaningful and satisfying living in the community is encouraged. Services are typically provided in homes, schools, hospitals, or other community settings.

We also provide Case Management Services for Adults.

Available Services

  • Administrative Case Management: primarily an office-based level of service which provides short-term assistance to children and adolescents and their families in accessing community resources such as:
    • Housing
    • Financial Entitlements
    • Educational Services
    • Peer/Support Groups
    • Treatment and Rehabilitation Service

This service also monitors individuals receiving inpatient care at residential treatment facilities and tracks all outpatient treatment commitments to Penn Foundation.

  • Resource Coordination: provides more intensive community-based assistance to children and adolescents with serious, persistent mental health disabilities and co-occurring substance abuse disorders and their families in achieving desired life goals and roles. Goals may include but are not limited to:
    • Keeping child with family in the community (rather than foster care or treatment facility)
    • Improving educational status
    • Increasing social support networks
    • Reducing behavioral health hospitalizations

  • Intensive Case Management: provides the most intensive community-based assistance to children and adolescents with serious, persistent mental health disabilities and co-occurring substance abuse disorders and their families. Like Resource Coordination, outcomes focus on the desired goals and life roles of the children and their families. Intensive Case Management offers more frequent contact and a 24 hour on-call support component.
