Case Management
Case Management provides individualized supportive services to adults, ages 18 and older, using a psychosocial rehabilitation philosophy. The focus of our services is to promote recovery through re-integration in the community. Our goal is to improve daily functioning and independence.
Services are provided in homes, hospitals, or other community settings. The primary objective of our services is to work cooperatively with adults to assist them in maximizing successful community living.
We also provide Case Management Services for Children and Adolescents.
Available Services:
- Administrative Case Management: provides short-term assistance to adults in accessing community resources such as:
- Low-Income Housing
- Financial Entitlements
- Vocational Rehabilitation Services
- Support Groups
This service monitors individuals receiving inpatient care at the area state hospitals, residential treatment facilities, and tracks all outpatient treatment commitments to Penn Foundation.
- Resource Coordination: provides more intensive community-based assistance to adults with serious and persistent mental health disabilities; helps to:
- Maximize Independent Living
- Improve Vocational Status
- Increase Social Support Networks
- Reduce Psychiatric Hospitalizations
- Intensive Case Management: provides the most intensive community-based assistance to adults with serious, persistent mental health disabilities. Like Resource Coordination, outcomes focus on:
- Maximize Independent Living
- Improve Vocational Status
- Increase Social Support Networks
- Reduce Psychiatric Hospitalizations
Although this service is very similar to Resource Coordination, clients registered with Intensive Case Management have staff contact on a more frequent basis (at least bi-weekly) and are provided with 24-hour, seven-day-a-week case management crisis coverage.
